Register As Care Giver If you want to work for as caregiver please provide the below required information and register with us. Drivers’ License or Identification Card*Must submit for proper identificationWork Permit or Resident Permit*Must be submitted for work eligibility purposesSocial Security Card*Must submit for identification and work eligibility purposesCPR & First Aid Card*The requirement to work as a caregiver. Contact your local Red Cross for class schedules or you may also locate a private provider or instructor.Physical /TB Test*Must obtain proof that you are in good health before you have any contact with clients. You must take physical with any doctor, clinic or any hospital that you prefer. If you have done a physical form that has not exceeded a year from the date completed, you may use the same one.Live Scan / Fingerprinting*You may obtain a form for fingerprinting from our office for DOJ, FBI Clearance. You may use fingerprints completed for employment. We do not accept fingerprinting from INS.OthersDepending on the type of facility that you are being hired for, you may need to complete one or more of the following certifications: C.P.I, Direct Support Professional Year 1 & 2.
